Apply to be an affiliate

Rules:

1. Please try and leave a comment every week or so.
2. Your site must have some visitor content.
3. So i know you have read these put "cookie dough" in other.
4. I have the right to deny any application.
5. You must have my link or current button on your homepage/affies page.
6. I will accept any sites except myspace & such.

Form

Send a email to affiliateapply@beth-design.com (please make sure you spell this correctly as many people don't and then complain the email didn't send) and include your name, site, email and why you wanna affiliate.

If i don't reply within 2 weeks i haven't accepted, sorry!
